How to Refer a Patient to Receive Hearing Devices Through the Sound of Life Foundation

If you know of a patient who you think would be a good candidate to receive hearing aids through the Sound of Life Foundation, please let them know about us!

Patients are welcome to apply for hearing aids through SOLF, regardless of where in the U.S. they reside. SOLF works with providers throughout the U.S.

Qualifications:

Be 18 years of age or older
Have a treatable hearing loss
Must meet the SOLF financial guidelines (varies by family size – see below)
Be willing to pay the $350 processing fee ($175 per ear) at the time of application submission
Be willing to provide 36 hours of community service (12 hours to be completed before receiving hearing devices)

Use the Table shown to find your household size and income limit for determining financial qualification. If your taxable income is less than or equal to this amount, it appears you qualify financially for SOLF. *Applicant must meet all other qualifications for our program as well.
