Become a SOLF Provider

Changing Lives One Hearing Smile at a Time

We appreciate your interest in becoming a part of our SOLF Family as an official SOLF Provider!

The process is easy! We ask each provider to review and sign a Provider Agreement, which outlines what you can expect from your participation in Sound of Life Foundation. You can choose to complete this process online or download a PDF copy to sign and return by fax or email. We also require a copy of your professional license and a current certificate of insurance.

Once you have submitted all of the required paperwork to become a SOLF provider, you can expect to hear from us requesting a time to schedule your New Provider Orientation. This orientation is conducted over the phone and is an opportunity for us to better familiarize you with the program and answer any questions that you or your staff may have.
